About The Position

At Northland Heights, the LPN provides high-quality, compassionate care to residents while assisting with medication administration, monitoring health conditions, and supporting personal care staff. The LPN ensures residents’ safety, comfort, and wellness in accordance with state regulations and facility policies.

Requirements

• Current and valid LPN license in the state of PA.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and as part of a team.

Nice To Haves

• Prior experience in assisted living, long-term care, or geriatric nursing preferred.

Responsibilities

• Administer medications and treatments in accordance with facility policy.
• Monitor and document residents’ health status, vital signs, and any changes in condition.
• Coordinate and communicate with physicians, families, and management regarding resident care.
• Assist with developing and updating individual care plans based on resident needs.
• Supervise and support staff in daily care activities.
• Respond to emergencies and provide appropriate nursing interventions.
• Maintain accurate and timely documentation in medical records and reporting systems.
• Ensure compliance with all state, federal, and facility regulations and policies.
