About The Position
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team at a Skilled Nursing facility. The ideal candidate will have hands-on experience in direct patient care and be proficient in performing clinical nursing procedures.
Requirements
- Active LVN/LPN license in the state of Texas
- Minimum 1 year of direct patient care experience (2+ years preferred)
- Strong clinical and documentation skills
- Ability to work in a team-oriented, fast-paced environment
Responsibilities
- Provide direct patient care, including medication administration and treatments
- Educate patients on medications and care plans
- Maintain accurate and timely documentation of patient information
- Perform computer data entry (training will be provided)
- Monitor patient conditions and report any changes to the healthcare team
- Assist with and perform advanced nursing skills as required
