A Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient’s needs. As a Licensed Practical Nurse (LPN) on our Inpatient Rehabilitation Facility (IRF) team, you are an essential part of a collaborative mission to restore independence and improve lives. Backed by Penn Medicine Lancaster General Health and Lifepoint Rehabilitation, our 59-bed inpatient hospital provides an advanced, patient-centered environment where your contributions make a lasting impact. Our team works together to treat individuals recovering from stroke, brain injury, spinal cord injury, neurological disorders, orthopedic conditions, amputations, and complex medical conditions. With a focus on dignity, compassion, and innovation, we help our patients achieve their highest level of independence and quality of life.

Requirements

  • Graduate of an accredited Licensed Practical/Vocational Nurse program
  • CPR/BLS certification required
  • Current state licensure as a Licensed Practical/Vocational Nurse.
  • Minimum six months of Medical/Surgical experience in an acute care setting preferred.

Nice To Haves

  • IV Certification is highly preferred or must be obtained within 6 months.
  • Demonstrates strong clinical skills and commitment to safe, patient-centered care.
  • Ability to work effectively in an interdisciplinary team environment.

Responsibilities

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.
