About The Position

The Home Health LPN will provide intermittent care to Home Health and/or Hospice patients under the guidance of a Registered Nurse and according to an established physician Plan of Care, within the Licensed Practical Nurse scope of practice. Skills and duties may vary dependent upon your department or unit. Other duties may be assigned as required.

Requirements

  • Licensed Practical Nurse in LPN Program - Licensed Practical Nurse
  • 1 or more years of experience
  • Licensed Practical Nurse (LPN) - Single State - State Board of Nursing
  • Active Licensed Practical Nurse (LPN) licensure in accordance with the Kansas Nurse Licensure Compact
  • Healthcare Provider CPR – (American Heart Association or American Red Cross ONLY)

Nice To Haves

  • 3 or more years of experience

Responsibilities

  • Consistently meets work schedule.
  • Performs a comprehensive assessment to provide the most effective patient care for a given population in the home environment.
  • Accurately documents and maintains all elements of the patient record according to department policies on timeliness and clinical content.
  • Effectively communicates to clinicians, physicians, patients, patient caregivers and RN Case Managers adhering to departmental process and regulations for all forms of communication.
  • Follows established Plan of Care within scope of practice and updates RN Case Manager about changes in patient status, medications, patient teaching, visit frequency and treatments as indicated.
  • Maintains a safe environment for patients as demonstrated through adhearing to fire safety measures and fall risk measures as established by Home Care Standards.
  • Demonstrates effective clinical, critical thinking skills as demonstrated in application of disease management protocols, pain management interventions and clinical equipment according to department processes, policies and scope of practice in the Home Care setting.
  • Considers patient's age, psychosocial, spiritual and cultural needs to therapeutically meet patient and patient caregiver needs as demonstrated in implementation of patient centered goals and interventions.
  • Participates and seeks out learning experiences.
  • Seeks out professional expertise to improve patient care.
  • Participates in IDT and department in services.
  • Seeks out resources in efforts to provide solutions in the home setting.
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.
