About The Position

We are seeking a Licensed Practical Nurse (LPN) to join our Nursing team and provide compassionate, high-quality care to residents in a mission-driven, person-centered environment. This role supports clinical care, resident advocacy, and team collaboration while ensuring dignity, safety, and quality outcomes.

Requirements

  • Licensed Practical Nurse (LPN) in good standing with the Minnesota Board of Nursing
  • Current CPR and First Aid certification
  • Strong clinical observation and assessment skills
  • Effective communication for collaboration with residents, families, and care teams
  • Eligible to work in federally funded healthcare programs
  • Ability to stand, walk, and move throughout the facility during the workday
  • Adequate vision and hearing to perform nursing assessments and respond to resident needs
  • Ability to push, pull, and lift up to 50 pounds
  • Frequent bending, reaching, and stooping to assist residents
  • Ability to manage the mental and emotional stress associated with direct resident care

Nice To Haves

  • Preferred experience in long-term care, assisted living, skilled nursing, or memory care
  • Familiarity with EMR systems preferred
  • Knowledge of geriatric care and chronic disease management preferred

Responsibilities

  • Provide direct nursing care, including medication administration, vital sign monitoring, and routine treatments
  • Assist in developing and implementing individualized resident care plans
  • Observe residents and document health status, changes, and responses to treatment
  • Operate and monitor medical equipment to assess resident health
  • Supervise and support caregiving staff to ensure care plan adherence
  • Communicate resident condition changes to appropriate providers, staff, and families
  • Advocate for residents by educating families and caregivers on health and wellness strategies
  • Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care
  • Maintain accurate and timely documentation in EMR systems
  • Participate in infection control, quality assurance, and regulatory compliance initiatives
  • Foster a supportive, respectful, and resident-centered care environment
