About The Position

We are seeking a Licensed Practical Nurse (LPN) to join our Nursing team and provide compassionate, high-quality care to residents in a mission-driven, person-centered environment. This role supports clinical care, resident advocacy, and team collaboration while ensuring dignity, safety, and quality outcomes.

Requirements

Licensed Practical Nurse (LPN) in good standing with the Minnesota Board of Nursing
Current CPR and First Aid certification
Strong clinical observation and assessment skills
Effective communication for collaboration with residents, families, and care teams
Eligible to work in federally funded healthcare programs
Ability to stand, walk, and move throughout the facility during the workday
Adequate vision and hearing to perform nursing assessments and respond to resident needs
Ability to push, pull, and lift up to 50 pounds
Frequent bending, reaching, and stooping to assist residents
Ability to manage the mental and emotional stress associated with direct resident care

Nice To Haves

Preferred experience in long-term care, assisted living, skilled nursing, or memory care
Familiarity with EMR systems preferred
Knowledge of geriatric care and chronic disease management preferred

Responsibilities

Provide direct nursing care, including medication administration, vital sign monitoring, and routine treatments
Assist in developing and implementing individualized resident care plans
Observe residents and document health status, changes, and responses to treatment
Operate and monitor medical equipment to assess resident health
Supervise and support caregiving staff to ensure care plan adherence
Communicate resident condition changes to appropriate providers, staff, and families
Advocate for residents by educating families and caregivers on health and wellness strategies
Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care
Maintain accurate and timely documentation in EMR systems
Participate in infection control, quality assurance, and regulatory compliance initiatives
Foster a supportive, respectful, and resident-centered care environment
