About The Position

"Navitas Healthcare, LLC" is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF). The LPN will provide direct patient care, support daily nursing operations, and collaborate with Registered Nurses and interdisciplinary teams to ensure high-quality, patient-centered care and positive resident outcomes.

Requirements

Education: Completion of an accredited Practical Nursing program (Diploma or Certificate)
Successful completion of NCLEX-PN
Licensure & Certifications: Active LPN License (Required)
BLS Certification (Required)
COVID-19 Vaccination (Required)
Experience 0–2 years of nursing experience (Preferred)
Skilled Nursing Facility (SNF) or Long-Term Care experience (Strongly Preferred)

Nice To Haves

ACLS Certification (Preferred)

Responsibilities

Provide direct patient care in accordance with care plans and facility policies
Assist in implementing and evaluating individualized resident care under RN supervision
Administer medications and treatments as permitted by state regulations
Monitor patient conditions and report any changes to RNs or physicians
Collaborate with healthcare teams to support optimal resident outcomes
Participate in patient rounds and address resident and family concerns
Maintain accurate and compliant documentation
Assist with delegation of tasks and support CNAs as needed
Follow infection control, safety, and compliance standards
Participate in quality improvement initiatives and audits
Provide guidance and support to CNAs and junior staff
