As a Licensed Practical Nurse (LPN) , you’ll provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. Requirements
- Graduate of an accredited practical/vocational nursing program
- Current LPN/LVN license in the State of Arizona
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
Nice To Haves
- 3 years of nursing experience preferred (Home Health or MedSurg a plus)
Responsibilities
- Deliver compassionate, direct patient care in accordance with state regulations
- Implement care plans developed by the RN and document progress accurately
- Monitor patient conditions and communicate observations to the RN and physician
- Collaborate with the care team to ensure seamless, high-quality service
- Participate in professional development and team meetings
