Location This position is based in Pensacola, FL, supporting Covenant Home Health Care patients through our partnership with Concierge Home Care. Your Role as a Home Health Licensed Practical Nurse (LPN) As a Home Health LPN, you will provide high-quality, personalized nursing care directly in patients’ homes—supporting independence, recovery, and overall well-being. Working under the direction of a Registered Nurse, you will play a vital role in delivering compassionate, consistent care.

Requirements

  • Unencumbered Florida Licensed Practical Nurse (LPN) license (required)
  • Valid driver’s license, auto insurance, and reliable transportation (required)

Nice To Haves

  • Home health experience preferred, but not required

Responsibilities

  • Following individualized, goal-driven plans of care established by the RN
  • Collaborating with patients, caregivers, Covenant leadership, and a supportive Concierge clinical team
  • Utilizing dictation services to streamline documentation and maximize patient-facing time
  • Delivering hands-on nursing care, including:
  • Acute and chronic disease management
  • Medication administration and management
  • Post-surgical rehabilitation support
  • Pain management
  • Wound care
