About The Position

As a Licensed Practical Nurse (LPN), you perform nursing work in the care and treatment of 8 individuals with intellectual and developmental disabilities (IDD) and co-existing medical conditions at our Adult Intellectual and Developmental Disability (IDD) Intermediate Care Facility (ICF), which may include physical health issues/injuries and difficulty meeting their daily living activities. This Position Offers: - Full-Time schedules - 1st shift or 2nd shift-availability needed to work every other weekend. - A great opportunity for new LPNs or experienced nurses looking to work outside of a traditional medical office, hospital, or nursing home setting. - A small setting where you are able to build relationships with the individuals you support. - The ability to work 1 on 1 with individuals. - The opportunity to attend community outings with individuals and teammates. CSG’s Intellectual and Developmental Disability (IDD/ASD) Services is excited to offer a 2 week paid orientation for this position. Orientation includes the necessary foundational training needed before providing services in the programs and sets you up for a successful career at CSG. Orientation is held in-person at our Mountville, Williamsport, and Bethlehem offices. Mileage reimbursement is provided for orientation related travel.

Requirements

  • A valid Licensed Practical Nurse (LPN) license in the state of Pennsylvania.
  • Willingness to accompany individuals into the community and provide support and assistance in interactions while maintaining sensitivity to the rights and dignity of the individual.
  • Ability to work flexible hours to meet individual's and program's needs.
  • Must have a valid driver’s license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individuals to appointments.

Responsibilities

  • Evaluate, observe, and assess individuals' physical health conditions.
  • Work with and coordinate care with pharmacy, physicians, and other health care providers.
  • Assist individuals with any treatments prescribed by physician.
  • Ensure individuals receive error free medication at prescribed times.
  • Educate individuals on medication, physical health, psychiatric health, nutrition, and selfcare.
  • Educate coworker's on any needed physical health matter for consistency in care.
  • Train staff to correctly carryout physician's orders.
  • Provide medication/health teaching as needed.
  • Provide First/Aid/Acute care as needed.
  • Schedule and attend individuals' medical appointments.
  • Provide direct care as needed.
