About The Position

Avamere is seeking a Licensed Practical Nurse (LPN) for full-time and per diem positions at Avamere Olympic Rehabilitation of Sequim. The primary responsibility of an LPN is to implement and maintain established nursing objectives and standards, provide direct nursing care to residents, and supervise the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs). The role involves a variety of essential duties including basic nursing care, patient monitoring, collaboration with other healthcare professionals, and maintaining patient safety and accurate documentation.

Requirements

  • Must possess a nursing degree from an accredited college or university
  • Must possess a current, unencumbered, active license to practice as an LPN in this state
  • Must maintain and have an active CPR/BLS during employment
  • Experience in electronic medical records documentation

Nice To Haves

  • Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting

Responsibilities

  • Implement and maintain established nursing objectives and standards
  • Provide direct nursing care to the residents
  • Supervise the day-to-day nursing activities performed by the CNA’s
  • Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations
  • Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN
  • Perform wound care, including changing dressings and monitoring for signs of infection
  • Assist residents with personal hygiene, dressing, bathing, and mobility
  • Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary
  • Record a patient's history and current condition
  • Observe and document changes in a resident's condition, reporting any concerns to the RN
  • Communicate effectively with residents, families, and other healthcare professionals
  • Work under the supervision of an RN, following their guidance and instructions
  • Supervise certified nursing assistants (CNAs) in providing basic nursing care
  • Collaborate with other healthcare professionals to ensure residents receive comprehensive care
  • Maintain a safe environment for residents, following infection control protocols and other safety guidelines
  • Accurately document all care provided, including medications administered, vital signs, and any observations
  • Provide emotional support and comfort to residents and their families
