About The Position
Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. Team 7: Belle Terre, Centereach, Farmingville, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jeff Station, Setauket, St. James, Stonybrook, & Terryville. Weekend Day shift: 7:00AM to 5:30PM Provide 16-18 visits per weekend- ensuring coverage of high priority cases
Requirements
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
Responsibilities
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
