About The Position

We are seeking a Licensed Practical Nurse (LPN) to join our assisted living team in a primarily weekday, daytime role focused on resident-centered care and strong collaboration. This position operates under the supervision and guidance of an experienced Registered Nurse (RN), in accordance with Minnesota Assisted Living Licensure requirements (Minn. Stat. Chapter 144G). As part of a supportive, team-based care environment, the LPN plays a key role in medication management, monitoring resident health needs, contributing to care planning, and serving as a clinical resource for team members. This position includes participation in a shared on-call rotation. Most on-call situations involve providing clinical guidance and support by phone to on-site staff, with in-person responses uncommon. The rotation is designed to be reasonable, well supported, and includes additional compensation for on-call responsibilities. Requirements

  • Graduate of an LPN nursing program and licensed/registered in Minnesota with a current, unencumbered license.
  • Demonstrate sound judgment, decision-making, problem-solving, and the ability to work independently.
  • Exhibit leadership, dependability, and flexibility in changing situations.
  • Accurately administer medications and treatments.
  • Effectively use computers, tablets, and electronic records.
  • Communicate clearly in English, both verbally and in writing, and follow instructions accurately.
  • Use tact, strong listening, and customer service skills with residents, families, and staff, remaining calm and professional.
  • Work collaboratively as a team member and maintain a positive demeanor.
  • Apply knowledge of validation techniques, reality orientation, and appropriate activities for residents with dementia.
  • Must receive a “not disqualified” criminal background result from the Department of Human Services and not be on the OIG exclusions list.
  • Must be screened for TB.
  • Must have a current driver’s license.

Nice To Haves

  • Graduation from an accredited program.
  • Minimum 12 months’ experience in home care, long-term care, acute care, or public health.
  • Working knowledge of the state requirements for the Comprehensive Home Care license and Minnesota Housing with Services registration.

Responsibilities

  • Work under the direction of the RN to monitor resident needs and condition changes, and report findings promptly.
  • Report resident concerns, condition changes, and emergencies to the RN and initiate emergency procedures as needed.
  • Review RN-developed care plans, consult with the RN on resident conditions and treatments, and assist with implementing physician orders.
  • Accurately transcribe physician orders and maintain proper charting and documentation.
  • Assist with medication ordering, setup, and administration.
  • Support resident admissions, transfers, and discharges as directed by the RN supervisor.
  • Assemble, operate, and maintain resident care equipment (e.g., oxygen supplies).
  • Support, guide, and assist unlicensed staff to ensure timely, quality care delivery.
  • Assist the RN with orienting, training, mentoring, and evaluating unlicensed staff.
  • Attend required meetings, trainings, and in-service sessions.
  • Perform delegated tasks and other duties as assigned by the RN.
