About The Position

"Navitas Healthcare, LLC" is a compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) team for the evening shift. The LPN will provide hands-on nursing care, collaborate with interdisciplinary healthcare teams, and ensure the safety, comfort, and well-being of residents in a skilled nursing environment.

Requirements

• Active LPN license in Wisconsin
• Experience in Long-Term Care or Skilled Nursing preferred
• Strong clinical assessment, critical thinking, and communication skills

Nice To Haves

• Experience in Long-Term Care or Skilled Nursing preferred

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Provide direct resident care including bathing, feeding, toileting, and mobility assistance
• Monitor, document, and report resident vital signs, health status, and responses to care
• Collaborate with RNs, CNAs, and interdisciplinary staff to ensure resident-centered care
• Assist in developing and implementing individualized care plans
• Maintain infection control standards and ensure a safe care environment
• Serve as a safety companion for residents when required
• Participate in quality improvement initiatives to enhance resident outcomes
• Maintain accurate and timely clinical documentation
