As a member of the Health Support Center (HSC) team, you'll support those that are in our facilities who are interfacing and providing care to our patients and community members to positively impact our mission of making communities healthier ®. More about our team Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit, with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs), Licensed Practical Nurses (LPNS) Certified Nursing Assistants (CNAs), and Medication Aides. Operating with a patient ratio of 1:15 days, 1:19 evenings and 1:25 nights, our unit cares for patients with Congestive Heart Failure (CHF), COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.

Requirements

• Current state LPN license
• Degree from an accredited nursing school
• Basic Life Support certification is required within 30 days of hire

Nice To Haves

• Long-term care (LTC) preferred or desired to work with LTC patients

Responsibilities

• Accurately performs patient assessments and identifies patient needs
• Identifies and initiates appropriate nursing interventions
• Provides care appropriate to condition and age of the patient
• Performs timely and appropriate documentation relating to medical necessity in the medical record
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
• Performs timely and accurate QI assessments
