About The Position

At our Skilled Nursing Facility, RNs and LPNs play a vital role in helping older adults recover, maintain dignity, and live safely - whether they’re here for short-term rehab or long-term memory care. You’ll be part of a supportive, multidisciplinary team that values compassion, teamwork, and consistent quality care. The Terrace is a 115-bed skilled nursing facility in Waukegan, dedicated to providing high-quality care to older adults through short-term rehabilitation and long-term memory care. Our residents come to us following hospitalization, surgery, or illness, or because they need ongoing support related to dementia or chronic conditions. Clinical care at The Terrace is resident-centered and team-driven. Nurses, therapists, social services, and support staff work closely together to promote recovery, maintain function, and preserve dignity. We focus on safe transitions of care, individualized treatment plans, and meaningful outcomes for each resident. At The Terrace, clinicians are respected as professionals. Your voice matters, your clinical judgment is valued, and your work directly impacts patient outcomes every day. This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers. Each center is unique; offering you a variety of long-term career opportunities.

Requirements

  • Licensed Practical Nurse (LPN) license in IL
  • Prior floor nurse experience within a skilled nursing facility is strongly preferred
  • Experience with the post-acute patients – specifically the cardiac, post op ortho & the medically complex
  • Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

Responsibilities

  • Provide safe, person-centered nursing care tailored to older adults’ needs
  • Administer medications and treatments following individualized care plans
  • Monitor resident progress and promptly address changes in condition
  • Collaborate closely with CNAs, therapists, and physicians as part of the care team
  • Help maintain a calm, respectful, and safe environment for all residents
  • Document care accurately using electronic health records (PCC)
