About The Position

As an LPN, you'll provide hands-on nursing care directly in patients' homes, supporting individuals through acute, chronic, or rehabilitative health challenges. Under the supervision of a Registered Nurse (RN), you'll play a key role in delivering high-quality care, advocating for your patients, and ensuring their care plans are accurately followed and documented. This position will cover towns in Southern Fairfield County, including Stamford and Greenwich.

Requirements

  • Graduate of an accredited nursing program.
  • Active LPN license in the State of Connecticut.
  • Minimum of 2 years clinical nursing experience (home care experience strongly preferred).
  • Strong understanding of medical terminology and Medicare/insurance documentation requirements.
  • Calm under pressure with strong organizational and multitasking skills.
  • Excellent interpersonal communication and customer service skills.
  • Proficient with electronic health records and computer documentation.
  • Must have reliable transportation and the ability to travel in all weather conditions.

Responsibilities

  • Deliver skilled, direct patient care as assigned, while communicating timely updates to the RN Case Manager, physician, and Clinical Supervisor.
  • Monitor and update nursing care plans based on physician directives.
  • Educate clients and families about diagnoses, medications, and treatment plans.
  • Use and maintain medical equipment and nursing supplies properly and efficiently.
  • Document patient information accurately using electronic medical records.
  • Participate in the on-call rotation and provide support to other clinical staff as needed.
  • Identify and escalate any emerging clinical issues to the appropriate leadership.
  • Build strong, trust-based relationships with patients, families, and team members.
  • Support agency quality improvement efforts and participate in ongoing professional development.
