We are currently seeking a skilled and caring Licensed Practical Nurse (LPN) to join our Health Services team. As an LPN, you will play a vital role in providing high-quality, person-centered care while supporting residents’ physical, emotional, and overall well-being. Requirements
- LPN degree from an accredited nursing program with current state licensure
- CPR/First Aid certification (or ability to maintain per state requirements)
- Strong clinical, communication, and organizational skills
- Compassion, professionalism, and commitment to resident-centered care
- Ability to work rotating weekends and flexible hours as needed
Nice To Haves
- 1–2 years of experience working with older adults preferred
Responsibilities
- Provide direct nursing care within the LPN scope of practice
- Administer medications and treatments
- Observe, assess, and document resident conditions and care provided
- Communicate effectively with residents, families, and the healthcare team
- Support individualized care plans for Assisted Living and Memory Care
- Assist with training and supporting medication aides and new team members
- Model Touchmark values and uphold Gold Standards of care
