About The Position

Licensed Practical Nurse (LPN) I & II Enriched Housing at The Terrace at Glen Eddy Shift/hours/days – Part Time Evenings, (every other Saturday 3pm-11pm) Long-standing team looking to welcome an LPN. We pride ourselves on quality care and without the heavy, overburdened workload of a Skilled Nursing Home environment, we can enjoy our residents and preserve their quality of life, while working as a team. At The Terrace at Glen Eddy, we are a unique team, with low turnover and content staff. We are big on teamwork and making The Terrace a place where we have time to provide the care each resident deserves. No lifts, no feeding, no treatment carts! Every nurse and aide is trained to assist with medication and ADL’s, and in the assisted living environment, it’s a great place to hone your caregiving skills. We are a beautiful, small, 42 bed assisted living located in the heart of Niskayuna, convenient to Schenectady, Latham, and surrounding areas. Position Summary: LPNs (Licensed Practical Nurses) are responsible for medications, treatments, and staff oversight under the direction of the RN. In this position you will bring trust, comfort, and empathy to all resident interactions and make a positive difference in the lives of our residents.

Requirements

  • Must be a high school graduate.
  • Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
  • Must be licensed with current registration in New York.
  • Must be able to: Stand, walk, and lift for long periods of time
  • Read and write using medical terminology
  • Working knowledge of computers including electronic medication administration records, processing of orders, checking email, and preforming required inservicing, along with use of other required applications
  • Have the visual and auditory acuity adequate to perform observations and patient care
  • Maintain composure and positivity in a high stress environment
  • Promote physical, cognitive, and psychosocial well-being of each resident
  • Work with residents in a highly personal manner during stages of acute and chronic illness

Responsibilities

  • Supports the Trinity/SPHP Vision, Mission, Values in all encounters with residents and families
  • Adheres to the SPHP Code of Conduct
  • Demonstrates accountability as an individual and team member in meeting basic resident and family needs
  • LPNs provide direct and indirect care under the direction of an RN including procedures, treatments, and related duties as assigned and within the NYS Scope of Practice for an LPN.
  • Resident support – Deliver compassionate, person-centered care while fostering a sense of dignity and independence for each resident.
  • Medication administration – Conduct daily medication pass with accuracy and care in accordance with physician orders and facility guidelines.
  • Shift leadership – provide leadership for the care team during your shift, including guidance and support.
  • Team collaboration – work closely with resident assistants, other nurses, and management in a cooperative, respectful environment.
  • Consult with the RN on any changes in a care plan or schedule and communicate with patients and their families about their care plan.
  • Observe, measure, record, and report clinical data relating to each resident's health status within licensure requirements.
  • Attends and actively participates in in-servicing, orienting and educational activities.
