Open Position Currently looking for an experienced Home Health License Practical Nurse (LPN) to fill openings in North Phoenix, AZ and surrounding areas. Requirements
- Home Care experience preferred but not required
- LPN State License required
- Basic Life Support (BLS)
- Valid Drivers License
- Current Finger Print Clearance card
- 1 year or current TB test
- Reliable mode of transportation
- Malpractice Insurance

Responsibilities
- Effectively collaborate with care team
- Patient assessment and evaluation of patients progress towards goals
- Implementation of care plan
- Plan and facilitate appropriate discharge
- Accurate and complete patient documentation
- Provide care which promotes quality, continuity, and patient safety
- Provide skilled nursing care to patients in the community
- Compliance with Electronic Visit Verification (EVV)
