Open Position Currently looking for an experienced Home Health License Practical Nurse (LPN) to fill openings in North Phoenix, AZ and surrounding areas. Requirements

  • Home Care experience preferred but not required
  • LPN State License required
  • Basic Life Support (BLS)
  • Valid Drivers License
  • Current Finger Print Clearance card
  • 1 year or current TB test
  • Reliable mode of transportation
  • Malpractice Insurance

Responsibilities

  • Effectively collaborate with care team
  • Patient assessment and evaluation of patients progress towards goals
  • Implementation of care plan
  • Plan and facilitate appropriate discharge
  • Accurate and complete patient documentation
  • Provide care which promotes quality, continuity, and patient safety
  • Provide skilled nursing care to patients in the community
  • Compliance with Electronic Visit Verification (EVV)
