About The Position

Job Title: Licensed Practical Nurse (LPN) Location: Colorado Military Health System (CMHS), Fort Carson, CO, 80913 Tentative Start Date: December 1, 2025 Work Schedule: Monday–Friday, 8-hour shifts between 0600–1700, excluding federal holidays Qualifications: Education: Graduate of a practical nursing program accredited by a U.S. agency. Experience: Minimum of 3 years of clinical nursing experience within the last 5 years. Experience in referral management, TRICARE systems, or military healthcare environments highly preferred. License/Certification: Current, full, active, and unrestricted LPN license in a U.S. jurisdiction; BLS certification required. Other Requirements: Must be a U.S. Citizen. Proficiency with Microsoft Office (Word, Excel, Access) and EHR systems (GENESIS preferred). Strong communication, organizational, and customer service skills. Knowledge of patient privacy, HIPAA regulations, and medical ethics. Documents Required for Submission: Updated Resume (With all gaps explained) Current, full, active, unrestricted license as a Licensed Practical Nurse in any U.S. state or territory. Basic Life Support (BLS) certification.

Responsibilities

  • Provide direct patient care under the supervision of physicians and Registered Nurses (RNs).
  • Perform patient assessments, document vital signs, administer medications, and update patient records.
  • Support the Referral Management Team by processing, tracking, and coordinating specialty care referrals.
  • Educate patients and families on treatment plans, medication use, and health benefits.
  • Assist in analyzing referral and utilization data, preparing reports, and ensuring compliance with TRICARE and DHA policies.
  • Act as a liaison between patients, providers, and referral management centers to ensure continuity of care.
  • Maintain accurate nursing and referral records, ensuring timely data entry into EHR systems.
  • Demonstrate knowledge of utilization management and review (UM/UR) principles when supporting referral workflows.
  • Provide consultation and administrative support for TRICARE beneficiaries.
