The Opportunity The Licensed Practical Nurse (LPN) will provide support to care management teams, leadership, and patients, as a part of Rhode Island Primary Care’s (RIPCPC) interdisciplinary care team. The LPN works with the Nurse Care Management Team and primary care practices to facilitate and / or provide patient education and care coordination in a patient centered care model.

Requirements

  • High School Graduate
  • Licensed LPN, state of RI
  • 3-5-year clinical work experience
  • Experience with Microsoft Office – Excel, PowerPoint, Word
  • Effective oral and written communication skills
  • Skilled in-patient engagement strategies and techniques
  • Skilled in telephonic interactions
  • Experience with customer service
  • Highly self-motivated
  • Flexibility and ability to shift work, learn new responsibilities and prioritize based on organizational need
  • Solid understanding of healthcare system and how to assist patients in navigation

Nice To Haves

  • EPIC Trained
  • Bilingual – Spanish preferred
  • Experience working in interdisciplinary teams
  • Experience with pediatric patients

Responsibilities

  • Work collaboratively with RIPCPC staff including and not limited to, nurse care managers (NCM), pharmacists (RPh), social work teams, quality improvement specialists (QIS) team members, PCP office staff and providers
  • Ensures accuracy and complete documentation with outreach per department policy
  • Work collaboratively with practice office staff and physicians
  • Assist with nursing related projects or programs
  • All other tasks as designated by the Nursing Manager or Director of Nursing
  • Active participation in staff meetings as scheduled and education sessions as needed
  • Triage referrals and inpatient discharges to the appropriate Interdisciplinary Team Members
  • Provide telephonic support and patient education post hospital discharge
  • Schedules all initial patient appointments for Nurse Care Managers
  • Monitors transitions of care, manages direct referrals, and reviews population health reports
  • Communicates with provider offices to support and coordinate patient care
  • Assist care team and practices in closing open gaps in care for patient
