About The Position
We are currently offering a $2500 Sign on bonus to any new full-time employee hired before the end of February, 2026. Transitions Healthcare Autumn Grove Care Center is currently seeking a Licensed Practical Nurse (LPN) to join our team of talented caregivers. Requirements
- Must be a graduate of an accredited LPN nursing program
- Must have a valid LPN license in PA

Responsibilities
- Take and record vital signs, such as blood pressure
- Administer medications and treatments as ordered and report adverse reactions to medications or treatments
- Monitor patients and chart medical records accurately
- Report the condition of a patient back to RNs (Registered Nurses) and doctors
- Communicate openly and effectively with patients and families, while demonstrating compassion and friendly behavior
