LPN – Full-Time | Home Care Field Nurse | No Weekends
About The Position
Abernathy Home Care is a well-established home care agency based in Stoneham, MA. We proudly provide high-quality, compassionate care to seniors and veterans in the community. We are actively hiring a Full-Time Licensed Practical Nurse (LPN) for a field-based home care position. This role works under the supervision of an RN Nurse Manager and supports client care, caregiver oversight, and clinical quality in the field. This is an excellent opportunity for an LPN looking for consistent weekday hours, autonomy in the field, and strong clinical support.
Requirements
- Active Massachusetts LPN license (required)
- Experience in home care, hospice, or community health preferred
- Strong communication and organizational skills
- Ability to work independently in a field-based role
- Valid driver’s license and reliable transportation (required)
- Comfortable with EMR systems and mobile documentation
Nice To Haves
- Spanish-speaking a plus
Responsibilities
- Conduct in-home nursing visits for home care clients
- Support care plan implementation under RN supervision
- Provide caregiver supervision and field support
- Monitor and report changes in client condition
- Assist with incident reporting and follow-up
- Ensure compliance with home care regulations and clinical standards
- Maintain accurate and timely documentation in electronic systems
