Requirements

  • Active LPN license in the state of Wisconsin
  • Minimum of one year of LPN experience in a healthcare facility
  • CPR certification (American Heart Association or American Red Cross)
  • Strong clinical, communication, and critical thinking skills
  • Reliable, professional, and able to adapt in different care settings

Responsibilities

  • Provide nursing care to patients in skilled nursing and assisted living facilities
  • Administer medications and treatments as prescribed
  • Monitor patient conditions and report changes to RN or provider
  • Document care accurately and thoroughly in patient records
  • Collaborate with care teams to deliver quality outcomes and a positive patient experience
