Licensed Practical Nurse (LPN) Symbii Home Health and Hospice Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join our team! Flexible hours: As a home health and hospice licensed practical nurse, you make your own schedule based upon your needs and the needs of your patients. You can run an errand, pick up your kids, or go to an appointment between patients. Minimal Weekends: A nurse at Symbii works an average of 1 weekend every 1-2 months and does minimal visits during their on-call weekend. Consistent Hours: We are growing! As a home health and hospice nurse, you will never have a lack of the hours that you want. Freedom from being stuck in the hospital or a facility for long shifts: Typical workday for home health and hospice is 8-5, M-F. Direct, one-on-one patient care: Care based at home gives the LPN the opportunity to get to know each patient well, create trusting relationships, and care for them in a way that makes a difference. Your nursing care provides your patient the opportunity to improve health and independence! Learn new skills and enhance your existing skills: Utilize a variety of nursing skills such as wound care, IV infusions, injection and medication administration, and patient/caregiver education. Work together with an amazing team while still working independently: Your team members are never more than a phone call away. Responsibility for your patients’ care: As the LPN, you will help make decisions for your patients’ health and collaborate with the case manager, physician, family, and other members of the care team. Be a part of a large caregiving community: You will have the opportunity to develop relationships with other caregivers in the facilities and physician’s offices of your patients as you coordinate care. What will you do as an LPN at Symbii? Provide direct patient care as defined in State’s Nurse Practice Act. Implement plan of care initiated by the registered nurse. Provide accurate and timely documentation consistent with the plan of care. Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s). Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing. Requirements

  • Must possess a valid LPN license in the State of Idaho
  • Must hold a current CPR certification
  • Must hold a valid driver’s license and current auto insurance

Responsibilities

  • Provide direct patient care as defined in State’s Nurse Practice Act.
