As a Licensed Practical Nurse (LPN) at Triniti Home Health & Hospice, you will provide compassionate, high-quality nursing care to patients in their homes while supporting their health, comfort, and independence. Additionally, you will be floating in our South Metro area. Our ideal candidate thrives in a fast-paced environment, demonstrates strong clinical skills, and is committed to delivering exceptional patient-centered care. Position Type: Full-Time, benefit eligible position Wage Range: $28 - $34 / hour depending on experience Location: Floating in the South Metro area

Requirements

  • Current Minnesota LPN license required.
  • Strong communication, organizational, and multitasking skills.
  • Ability to build positive relationships with patients, families, and healthcare professionals.
  • Comfortable using computers and electronic clinical documentation systems.
  • Self-starter who works well independently and as part of a team.

Nice To Haves

  • Minimum 2 years of experience in geriatrics, home care, hospice, or a related setting preferred.
  • CPR/BLS certification preferred or willingness to obtain.

Responsibilities

  • Provide skilled nursing care to home health and hospice patients following individualized care plans.
  • Monitor vital signs, assess patient conditions, and report changes to the RN Case Manager or physician.
  • Administer medications and treatments as ordered and monitor patient responses.
  • Collaborate with families, care teams, and other healthcare professionals to coordinate patient care.
  • Maintain accurate and timely electronic documentation using clinical charting software.
  • Manage medications, supplies, and equipment in accordance with agency policies and procedures.
  • Participate in team meetings, case conferences, and ongoing training as required.
  • Deliver compassionate care while maintaining patient confidentiality and high customer service standards.
