About The Position

Our client provides a full range of mental and emotional health services within a residential setting of ten beds . Each participant receives individualized engagement and goal‑oriented support designed to enhance independence and overall well‑being. The Long‑Term Structured Residence (LTSR) program is built around policies and procedures that emphasize participant safety, clinical consistency, and person‑centered care. The Licensed Practical Nurse (LPN) plays a vital role in the daily oversight, coordination, and direct intervention of all health‑related needs affecting program participants. This position supports both physical and emotional health within a collaborative team environment, ensuring all residents receive compassionate, high‑quality care in accordance with physician orders and program standards.

Requirements

  • Current and valid Pennsylvania LPN license required.
  • Hepatitis B Titer (within 1 year)
  • Current physical (within 6 months)
  • 2-step TB (within a 1 year)
  • BLS
  • PA Criminal Clearance (within 1 year)
  • Strong communication, documentation, and teamwork skills.
  • Compassionate, patient‑focused approach with an understanding of diverse emotional and behavioral needs.

Nice To Haves

  • Previous experience in mental health, residential, or rehabilitation settings preferred.

Responsibilities

  • Maintain accurate nursing documentation and medication records
  • Educate residents about their medications and ensure compliance with physician orders, documenting all administration in the electronic Medication Administration Record (eMAR)
  • Provide comprehensive nursing care, including wound care, trach care, diabetic counseling , and other clinical interventions as needed.
  • Conduct health and emotional assessments to identify emerging needs and make recommendations for care adjustments.
  • Communicate effectively with other nursing, clinical, and program staff to ensure continuity of care; promptly report significant issues to the Nurse Manager and Program Director
  • Maintain strict confidentiality, avoiding discussions about residents in public or non‑private settings.
  • Perform additional duties as assigned to support the mission and operations of the program.
