About The Position

We are currently seeking a Licensed Practical Nurse (LPN) to join our Covenant Home Health team and provide skilled nursing care to seniors in Senior Living Communities. The Licensed Practical Nurse (LPN) will provide direct patient care, including administering medications, wound care, and assisting with daily activities, under the supervision of the RN Case Manager or Director of Nursing. This role involves traveling to Senior Living Communities across the Phoenix Valley, ensuring that residents receive high-quality, personalized care in a supportive and compassionate environment. Full-Time and PRN availability. Coverage areas: West Valley & East Valley

Requirements

  • Current Licensed Practical Nurse (LPN) in Arizona in good standing.
  • Valid driver’s license, reliable transportation, and current vehicle insurance.
  • Must pass background checks and meet health and safety requirements.
  • Strong knowledge of nursing procedures, medication administration, and patient care.
  • Ability to work independently and manage a patient caseload efficiently.
  • Ability to follow philosophy, objectives, policies, and procedures that enhance high standards of skilled nursing practices.
  • Excellent communication skills and the ability to work effectively with patients, families, and the healthcare team.
  • Must be able to travel between various Senior Living Communities across the Phoenix valley.
  • Knowledge of Home Health rules and regulations required.
  • Must be motivated, a self-starter, with a passion to serve seniors.

Nice To Haves

  • Previous experience in home health care, long-term care, or geriatrics is preferred.

Responsibilities

  • Follows specific physician orders for the administration of care as directed on the 485 plan of care.
  • Keeps abreast of new knowledge and information so that clients will be assured of receiving up‑to‑date methods of care. Participates in formal continuing education sessions and in-service sessions.
  • Maintains open relationships and communication with client’s, client’s families, physicians, and other HHA staff involved in the care of the client.
  • Follows all State and Federal rules and regulations as they pertain to the Home Health Center.
  • Responsible for actively ensuring that the rehabilitation program is tailored to help each client on the caseload achieve and maintain the highest level of self-care and independence, as outlined in the client's care plan.
  • Documents each home health visit in the medical record, including nurse's notes on significant observations regarding the resident's needs for and response to medications and treatments.
