LPN – 12-Hour Home Care Shift
About The Position

As the parent company of AmeriStaff Nursing Services, Sun Medical Equipment & Supplies, and RxIV Pharmacy, AmeriCare Medical delivers comprehensive, patient-centered care solutions designed to support patients across the continuum of care. We are currently seeking Licensed Practical Nurses (LPNs) to join our home health team serving the Metro Detroit area. In this role, the LPN will provide skilled, one-on-one nursing care in the patient’s home, supporting individualized treatment plans while promoting comfort, safety, and quality of life. This position will support a patient located in Farmington Hills and involves 12-hour shift-based home care services. The ideal candidate will have experience assisting patients with tube feeding, bowel programs, and Hoyer lift transfers. This is an excellent opportunity for an LPN seeking flexible scheduling, meaningful patient relationships, and hands-on clinical experience in a home care setting.
Requirements
- Active Licensed Practical Nurse (LPN) license in the State of Michigan
- Valid driver’s license and reliable transportation
- Ability to document care using tablet or smartphone-based medical software
- Successful completion of a criminal background check
- Authorization to work in the United States

Responsibilities
- Provide skilled nursing care in accordance with the patient’s plan of care
- Monitor patient condition and report changes to the supervising RN or physician
- Assist with specialized care including tube feeding, bowel programs, and mobility support
- Safely perform patient transfers using Hoyer lifts and other assistive equipment
- Maintain accurate and timely documentation using mobile medical software
- Collaborate with interdisciplinary care teams to ensure coordinated patient care
- Promote patient comfort, dignity, and independence in the home environment
