Requirements

  • Current LPN license in the state of Kansas
  • Strong clinical and communication skills
  • Ability to work independently in patient homes or facilities
  • Compassionate approach to endoflife care
  • Valid driver’s license and reliable transportation

Nice To Haves

  • Hospice or home health experience preferred, but not required

Responsibilities

  • Provide direct nursing care under the supervision of an RN Case Manager
  • Monitor patient symptoms, medication needs, and comfort levels
  • Perform routine nursing procedures in accordance with the plan of care
  • Document assessments, interventions, and patient status accurately and timely
  • Communicate changes in condition to the interdisciplinary hospice team
  • Provide education and emotional support to patients and their families
  • Participate in team meetings and support overall hospice care goals
