About The Position

The VCU Center for Advanced Health Management (CAHM) is a home base for an advanced comprehensive care model for qualifying patients with complex care needs, including older adults. We are a specialized adult primary care clinic that is made up of two clinics: complex care and geriatric care. Our complex clinic cares for adults with multiple comorbidities and our geriatric clinic has geriatricians who specialize in dementia, mobility issues and caregiver burden. Our team supports a very diverse patient population so our nurses are always learning and growing. We are a wonderful team of nurses, medical assistants, social workers, pharmacy, x-ray, outreach and lab excited to welcome a new team member! The LPN job commences to integrate technical skills and theoretical knowledge to meet the needs of the patient and family. This role assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. This role also provides direction, education, and begins to assume leadership responsibilities at the unit level. The LPN job delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This role safely administers prescribed medications, identifying incompatibilities and/or interactions.

Requirements

  • Licensure, certification or registration varies depending on the incumbent`s employment background and transitional assignment
  • LPN licensure in Virginia or eligible Licensure
  • Current LPN Licensure in Virginia
  • AHA BLS Certification or equivalent
  • Varies based on transitional duty assignment.
  • Graduate from a Board of Nursing approved School of Practical Nursing
  • Physical: Lifting 50-100 lbs.
  • Other: Exposure to potentially hazardous and infectious substances
  • Activities: Prolonged standing, Prolonged sitting, Frequent bending, Walking (distance), Climbing (steps, ladder, other), Reaching (overhead, extensive, repetitive)
  • Mental/Sensory: Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking
  • Emotional: Steady pace, Frequent and intense customer interactions, Noisy environment, Able to adapt to frequent, change

Nice To Haves

  • Experience as a LPN in either an acute care setting or as applicable to area of practice

Responsibilities
