About The Position

The LPN is responsible for traveling to a patient/client's place of residence to provide skilled nursing care. The LPN is responsible for maintenance and accuracy of electronic medical records and coordination of care with other team members. The LPN works under the supervision of the Registered Nurse (RN) as a member of a team to give direct nursing care to patient/clients within the policies of the agency. The LPN will provide services within the scope of Federal and State regulations, licensure, policies and procedures of the agency. Schedule Mondays-Fridays; 8 hours a day; 30 visits per week. Employee may be required to work varying hours as dictated by staffing levels, service demands, or operational needs

Requirements

  • LPN in good standing with discipline specific licensing/certification agency and able to meet all federal and state licensure requirements.
  • Current CPR certification or ability to obtain within 90 days of hire.
  • Basic level knowledge of EMR (electronical medical records) and email functions.
  • Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.

Responsibilities

  • Assess and document patient/client needs and observation of patient's condition at each visit; report changes and concerns timely to RN; follow up with attending physician as directed.
  • Provide direct quality skilled nursing care, either directly or through RN supervision.
  • Provide education, support and teaching of care to the patient/client, family or other caregivers.
  • Maintain quality charting and records as required by agency policy and federal and state regulations.
  • Coordinate services and visits with patient/clients and/or family caregivers, as well as other members of the home health team.
  • Be knowledgeable of payer source guidelines.
  • Provide Hospice continuous care if needed.
  • Must have valid Driver's License
