About The Position
"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care to geriatric residents in a long-term care setting. This role involves medication administration, clinical treatments, patient monitoring, and collaboration with the healthcare team to ensure safe and effective care delivery.
Requirements
- Active LPN license (state-specific)
- Knowledge of geriatric and long-term care practices
- Strong clinical and assessment skills
- Ability to administer medications and perform nursing treatments
- Excellent communication and teamwork abilities

Responsibilities
- Administer medications and perform treatments within scope of practice
- Provide specialized care including wound care, catheter care, tracheostomy care, and enteral feeding
- Monitor and report changes in residents’ physical and behavioral conditions
- Assist with admissions, assessments, MDS, and care plan development
- Maintain accurate and timely clinical documentation
- Communicate effectively with residents, families, and interdisciplinary teams
- Participate in shift handoffs to ensure continuity of care
- Supervise CNAs and support staff; assist with ADLs as needed
- Ensure availability of necessary medical supplies and medications
- Participate in quality improvement initiatives and staff training
- Adhere to all regulatory standards, policies, and procedures
