Requirements

Currently hold an LPN license (GA or MULTI-STATE)
Hold a valid driver's license, auto insurance, and a clean driving record
Have a smart phone and email account
Understand the importance of ethics and client confidentiality
Applicants must be willing to complete all necessary requirements for employment

Responsibilities

Home Supervisory visits
Home consultations
New Client Intakes
Caregivers/employees supervision
Oversee all aspects of PCA Tests administration
Skilled Nursing for medically fragile clients under the GAPP Program
Assess, monitor, document and report progress of patients’ health and condition using required documents such as clinical progress notes
Carrying out a plan of care that best supports your client
Creating compassionate and supportive solutions to your client’s unique needs by collaborating with your Home Helpers associates and your client’s family
Other duties as assigned
