Position Summary Responsible for providing high quality, resident centered nursing care within our Skilled Nursing and Rehabilitation neighborhoods. Key Responsibilities LPNs play a vital role in delivering compassionate, hands on care while supporting residents’ health, comfort, and dignity. This role collaborates closely with RNs, physicians, and other care team members to ensure the highest standards of care are consistently met. At Still Hopes, LPNs are not alone. Responsibilities

  • Administering medications and treatments in accordance with physician orders
  • Monitoring residents’ conditions and reporting changes to the care team
  • Accurately documenting care, treatments, and resident progress
  • Ensuring residents’ physical, emotional, and social needs are met
  • Communicating regularly with residents and their families, providing updates and support
  • Supervising and guiding nursing assistants in their daily responsibilities
  • Promoting a safe, clean, and supportive environment for residents and staff
