About The Position

The Licensed Practical Nurse (LPN) provides direct oversight and support to the care team on-site through hands-on education, coaching, and modeling. This position ensures the health and well-being of participants through advocacy with medical providers, coordination with visiting nursing services, and participation in discharge planning following hospital or rehab stays. The LPN supports the maintenance of a safe environment by conducting medication and chart audits and collaborates closely with nursing and supervisory teams to meet individual needs.

Requirements

  • Valid driver’s license, reliable vehicle, sufficient auto insurance, and acceptable driving record.
  • Must be a Licensed Practical Nurse (LPN) with an active Massachusetts license.
  • Strong ability to work independently while prioritizing multiple tasks efficiently.
  • Proficient written, verbal, and organizational skills.
  • Adaptability to meet the changing needs of program participants.
  • Demonstrated compassion, patience, and strong interpersonal skills.
  • Must be able to obtain and maintain certifications in MAP, CPR, and First Aid.
  • Must be at least 18 years old.

Responsibilities

  • Communicate with participants and team members in a respectful, enthusiastic, and empowering manner that promotes self-esteem and self-direction.
  • Provide and model safe execution of health, safety, and medical protocols for participants. Offer training, observation, and review of protocol implementation to support staff, informing the Nurse Manager of concerns as needed.
  • Act as a liaison between participants and external providers including hospitals, clinics, and families, while promoting self-advocacy.
  • Monitor participants for medication side effects, infection, or symptoms of medical conditions and respond according to established protocols, including timely documentation and tracking.
  • Respond to medical or behavioral emergencies in accordance with MHA, MRC, and DDS policies. Ensure appropriate communication and follow-up with supervisory staff and emergency personnel as needed.
  • Report and document all suspected cases of abuse or neglect following mandated reporting procedures.
  • Assist in scheduling, coordinating, and, when appropriate, attending medical, dental, and psychiatric appointments. Administer medications per MAP standards and record health-related data as necessary.
  • Conduct medication log audits to ensure MAP compliance and accurate recordkeeping.
  • Participate in team meetings, supervisions, and ongoing training to enhance knowledge, skills, and collaboration with colleagues.
  • Maintain professionalism, strong interpersonal skills, and organizational capabilities to ensure participant goals and program outcomes are successfully met.
