Performs nursing assessment, treatment, education and health care service coordination to individuals and families. The LPN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services. Services will be provided at Community Healthlink and outreach locations.

Requirements

  • Grade N02: Community LPN: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
  • Must be able to pass a CORI background check.

Nice To Haves

  • 2-4 years of experience in medical nursing.

Responsibilities

  • Assesses the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, medication administration and documentation.
  • Provides appropriate health care and coordinates referrals as prescribed by providers.
  • Maintains documentation required for all primary care clients receiving in-home nursing services.
  • Promotes education and adherence to prescribed treatment protocols.
  • Communicates with health care facilities when clients are admitted and plans for discharge to ensure continuity of care.
  • Facilitates wellness education to individuals and offer groups with health and wellness focus.
  • Records all client information within electronic medical record system daily, received by outside healthcare agencies.
  • Complies with established division and program policies, procedures, and objectives.
  • Attends variety of meetings, conferences, and trainings as required or directed.
  • Demonstrates use of quality improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches and contributes to maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.
  • Maintains regular, reliable, and predictable attendance.
  • Maintains confidentiality.
