This position provides basic nursing care by assisting in the assessment of residents upon admission and on an ongoing basis. The LPN is responsible for remaining knowledgeable about and communicating any significant changes in a resident's condition to the appropriate interdisciplinary team (IDT) member, and accurately documenting all assessments. The role involves participating in the care planning process, updating care plans as needed, and communicating these plans to IDT members, residents, and their families. The LPN implements necessary nursing interventions, including medication administration and treatments, ensuring they are performed safely and timely. Administration of IV fluid is required if IV certified. The LPN must use critical thinking skills for changes in residents' conditions, notify physicians using the SBARR tool, and maintain composure during crisis situations. Monitoring resident care for effectiveness, identifying problem areas, and utilizing the QAPI process for resolution are also key. The LPN must be aware of residents' status to quickly identify behavioral changes or other untoward signs/symptoms, answer call lights, and assist with residents' ADLs as needed. A minimum of twelve hours of nursing-related continuing education is required, along with a commitment to staying current with best nursing practices and Bethesda's policies.
