Requirements

  • Current and active LPN license in good standing.

Responsibilities

  • Collaborate in dynamic team huddles to promote seamless communication.
  • Obtain comprehensive health histories and vital signs; prepare patients and assist providers with comfort and professionalism during exams and procedures.
  • Collect and document critical health care data, both in person and over the phone.
  • Support mental wellness by reviewing and entering PHQ-9 questionnaires.
  • Provide age-appropriate, culturally sensitive nursing care guided by compassion and best practices.
  • Serve as a patient advocate, reliable resource, and key contact throughout their care journey.
  • Ensure accurate documentation of subjective and objective data in the EMR system.
  • Administer medications and assist in proactive medication management.
  • Perform lab and diagnostic testing with precision and urgency, reporting critical results promptly.
  • Coordinate referrals and follow-up appointments with efficiency and empathy.
  • Facilitate coordinated, high-quality care in partnership with physicians, nurses, and the broader health care team.
  • Uphold strict confidentiality standards in line with HIPAA and infection control protocols.
  • Contribute to a collaborative, respectful team environment, promoting open communication and effective problem-solving.
  • Maintain universal precautions and safety standards for patient and staff well-being.
  • Stay ahead of the curve with ongoing professional development and a commitment to nursing excellence.
