Requirements

Current and active LPN license in good standing.

Responsibilities

Collaborate in dynamic team huddles to promote seamless communication.
Obtain comprehensive health histories and vital signs; prepare patients and assist providers with comfort and professionalism during exams and procedures.
Collect and document critical health care data, both in person and over the phone.
Support mental wellness by reviewing and entering PHQ-9 questionnaires.
Provide age-appropriate, culturally sensitive nursing care guided by compassion and best practices.
Serve as a patient advocate, reliable resource, and key contact throughout their care journey.
Ensure accurate documentation of subjective and objective data in the EMR system.
Administer medications and assist in proactive medication management.
Perform lab and diagnostic testing with precision and urgency, reporting critical results promptly.
Coordinate referrals and follow-up appointments with efficiency and empathy.
Facilitate coordinated, high-quality care in partnership with physicians, nurses, and the broader health care team.
Uphold strict confidentiality standards in line with HIPAA and infection control protocols.
Contribute to a collaborative, respectful team environment, promoting open communication and effective problem-solving.
Maintain universal precautions and safety standards for patient and staff well-being.
