About The Position

The Licensed Practical Nurse (LPN) delivers high-quality, compassionate direct patient care in the home, community, and care-center settings. This role supports patients and families through skilled symptom management, emotional support, and exceptional service aligned with the organization’s mission of providing dignified, patient-centered hospice care.

Requirements

• Current Ohio LPN license
• Minimum 2 years LPN experience (Med/Surg, Oncology, or Community Health; hospice preferred)
• Valid driver’s license and reliable transportation
• Strong communication and time management skills
• Ability to work independently

Responsibilities

• Provide compassionate, high-quality hospice and palliative nursing care through home visits, inpatient care, end-of-life/continuous care, and admissions support
• Deliver care in accordance with individualized Plans of Care and physician orders
• Monitor patient condition and promptly communicate changes to the supervising RN
• Educate patients and families on hospice services, insurance benefits, and care expectations
• Coordinate care transitions and order necessary Durable Medical Equipment (DME)
• Obtain and accurately document physician orders in the EMR
• Complete timely, compliant documentation per agency and regulatory standards
• Collaborate with interdisciplinary team members to ensure seamless, patient-centered care
• Participate in evening and weekend rotations as scheduled
• Perform additional duties as assigned to support patient care and team operations
