About The Position
The Licensed Practical Nurse (LPN) provides direct patient care in a skilled nursing setting while supporting and supervising nursing assistants. This role ensures high-quality care delivery in compliance with all applicable regulations and standards.
Requirements
- Graduate of an accredited Practical Nursing program
- Active LPN License (Massachusetts or Compact) – Required
- BLS / CPR / AED Certification – Required
- Experience in long-term care or skilled nursing preferred
- Strong clinical and patient care skills
- Effective communication and teamwork abilities
- Leadership and supervisory experience preferred
- Strong documentation and organizational skills
- Ability to work in a fast-paced environment and handle emergencies

Responsibilities
- Supervise and support nursing assistants in daily care activities
- Administer medications and treatments as prescribed
- Monitor patient conditions and document care accurately
- Implement and transcribe physician orders
- Assist with admissions, transfers, and discharges
- Perform clinical procedures (wound care, catheterization, tube feeding, etc.)
- Collect and process lab specimens
- Record vital signs and patient observations
- Communicate effectively with physicians and care teams
- Participate in care planning and interdisciplinary coordination
- Educate patients and families on care plans
- Ensure adequate medical supplies and equipment
- Maintain patient confidentiality and regulatory compliance
- Respond promptly to patient needs ensuring safety and comfort
