About The Position

The Licensed Practical Nurse (LPN) at RHD is responsible for overseeing all aspects of medical care for individuals served by the organization. This role is part of a multidisciplinary team providing holistic, values-based services to promote integrated physical and mental health wellness.

Requirements

  • Licensed Practical Nurse (LPN) license.
  • Current license to practice in the State of Delaware.
  • Valid driver’s license.

  • Ability to lift up to 50 pounds occasionally (medium physical demand).
  • Comfortable working in varied conditions (e.g., exposure to heat, cold, dust, fumes, confined spaces).
  • Physical abilities include standing, sitting, walking, climbing, reaching, kneeling, and repetitive motion.
  • Strong communication and documentation skills.
  • Commitment to providing culturally competent care.

Responsibilities

  • Manage medication ordering, delivery, storage, and administration per program guidelines.
  • Educate individuals and staff on health concerns; provide staff training as needed.
  • Assess and respond to new or ongoing health issues.
  • Complete required documentation for RHD and regulatory bodies.
  • Conduct screenings for depression, trauma, substance use, and criminal risk using validated tools.
  • Provide recovery-oriented, evidence-based treatment and collaborative documentation.
  • Act as liaison for medical care and community supports.
  • Coordinate care with prescribers and other providers; administer IM injections per physician orders.
  • Monitor vital signs and medication side effects; report findings to the team.
  • Ensure consumers have primary care providers, annual physicals, dental exams, and prescription schedules.
  • Participate in on-call rotation for 24-hour emergency and crisis intervention.
  • Develop recovery plans and recommend health objectives.
  • Deliver culturally competent, individualized services.
  • Perform other duties as assigned by the Team Leader/Program Director.
