About The Position
$5,000 Sign on Bonus - Work where you live! Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. Team 2 Covers ; Baldwin, Cedarhurst, Bellmore, Freeport, Inwood, Island Park,Lido Beach, Long Beach, Lynbrook, Malverne, Merrick, Oceanside, Point Lookout, Rockville Centre, Roosevelt, Valley Stream, Woodmere. 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $5,250 annually Additional compensation for visits over your caseload Job Details Our Home Care LPN’s: Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. Measure vital signs, Administer IVs, change dressings, clean wounds as needed Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
Requirements
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
Responsibilities
