About The Position

The Licensed Practical Nurse (LPN) at Wellstar MCG Health is responsible for providing direct patient care under the supervision of a Registered Nurse. This role involves managing the total needs of patients with medical and surgical requirements, ensuring high-quality care, and participating in patient education. The LPN also plays a key role in coordinating care, documenting patient information, and contributing to staff education and professional development.

Requirements

• High School Diploma or equivalent.
• Lic Practical Nurse certification required upon hire.
• BLS - Instructor or Basic Life Support certification required upon hire.
• Minimum 1 year medical/surgical nursing practice in an acute care environment preferred.
• Strong assessment and clinical skills.
• Positive leadership and problem-solving skills.
• Strong organizational and time management skills.
• Strong written and verbal communication skills.

Nice To Haves

• Experience with home health patient software and computer-based learning modules.

Responsibilities

• Assess and assist with managing patients' total needs following physician orders with RN oversight.
• Perform routine nursing procedures to meet the needs of the patient under the supervision of RN.
• Coordinate care and services with RN and other team members.
• Inform RN, therapist, and/or clinical supervisor of changes in patients' condition, as appropriate.
• Collaborate with RN to develop and revise individual plan of care based on patient need and physician order.
• Serve as patient educator to foster self-care or caregiver support for the home patient.
• Complete required documentation accurately within established timeframe.
• Participate in care team conferences with co-workers.
• Participate in the orientation and education of new employees and nursing students as appropriate.
