About The Position
The Licensed Practical Nurse (LPN) at Wellstar Family Medicine - West Point plays a crucial role in providing patient care under the supervision of a Physician, Nurse Supervisor, and/or Practice/Office Manager. This position requires the LPN to recognize patient needs and symptoms proactively, ensuring that care is delivered effectively and compassionately. The LPN is responsible for preparing patients for examinations, assisting during treatments, coordinating necessary tests, and ensuring follow-up care is provided. Additionally, the LPN is tasked with recording treatment details in the medical record and assisting with patient and family education. The role demands a high level of competency and skill to cater to patients across all age groups, including neonates, infants, children, adolescents, adults, and geriatric populations. The LPN is expected to perform other duties as necessary, maintaining a commitment to the standards set forth by the Georgia Professional Nurse Practice Act and the National Federation of Licensed Practical Nurses, Inc. (NFLPN). As a member of the patient services team, the LPN is expected to uphold the voice of the patient while adhering to system policies and procedures, supporting service excellence goals. The position emphasizes professional practice and responsibility, where the LPN must practice within the scope of nursing standards and promote health through evidence-based practices. The LPN is also responsible for coordinating care through effective partnerships, ensuring that patient care is individualized and optimal outcomes are achieved. Teamwork and collaboration are essential components of this role, as the LPN coordinates the delivery and documentation of safe, quality patient care. This includes engaging in interdisciplinary communication methods and promoting a respectful, inclusive clinical environment. The LPN is expected to demonstrate fiscal responsibility and contribute to the recruitment and retention of healthcare team members. Professional development is a key aspect of the LPN's responsibilities, which includes completing ongoing competency assessments and serving as a mentor for other practical nurses and unlicensed staff. The LPN is also involved in promoting evidence-based practice and participating in shared decision-making groups to enhance nursing practices. In terms of practice operations, the LPN provides direct patient care as directed by the provider and proactively plans for patient care across the continuum. The LPN also serves as a patient liaison and advocate, ensuring that care is delivered in a culturally sensitive manner and that the patient's goals are prioritized in the care plan.
