About The Position

The Licensed Practical Nurse (LPN) is responsible for the delivery of patient care utilizing the nursing process through data collection and assessment, identifying and determining the priority of patient's problems/needs, and evaluating the process and outcome of nursing care. The LPN coordinates, provides, and directs patient teaching activities and coordinates the care provided by health team members. The LPN complies to all documentation standards within the scope of the Medical Assistant and the LPN role. We are seeking a dedicated and compassionate LPN to conduct Medicare annual wellness visits in various Toledo Region locations. This role involves traveling short distances to designated facilities, providing high-quality care, and ensuring compliance with Medicare guidelines. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to improving patient health outcomes.

Requirements

  • Current Licensed Practical Nurse (LPN) from the state of employment or LPN applicant (required)
  • BLS certification is preferred upon hire and required within 30 days and/or prior to independent patient care
  • 1 year of clinical patient care experience (preferred)
  • EPIC Electronic Health Record (EHR) training (preferred)

Responsibilities

  • Conduct comprehensive annual wellness visits for Medicare patients, including assessments, screenings, and health education.
  • Review patient health history and medications, documenting findings accurately in electronic health records (EHR).
  • Develop personalized care plans in collaboration with patients and their healthcare providers.
  • Educate patients on preventive care, chronic disease management, and lifestyle modifications.
  • Ensure compliance with Medicare regulations and guidelines during all visits.
  • Maintain accurate records of patient interactions and follow-up actions.
  • Coordinate with other healthcare professionals as needed to ensure continuity of care.
  • Travel to various locations, adhering to a schedule that maximizes patient care and efficiency.
  • During non-clinical time: outreach to schedule patients for Medicare annual wellness visits, schedule hospital follow up appointments, completing transitional care management calls, patient outreach for preventative care to patients that are due, help practices with patient calls and inbasket messages if needed, along with other administrative duties as requested.
