About The Position

Westminster-Canterbury on Chesapeake Bay is currently hiring a Licensed Practical Nurse (LPN) Team Leader for Skilled Nursing, prehab, and rehab. We have an immediate opening for Full Time Days (7am - 7pm), Full Time Nights (7pm - 7am), and PRN (As needed). The schedule is 3 days one week, 4 days the next with an every other weekend rotation for all Full-Time positions.

Requirements

• Current LPN licensure in the state of Virginia.
• Current CPR certification.

Responsibilities

• Plans and coordinates clinical resident care and nursing personnel on a designated unit.
• Ensures that team is utilizing all opportunities for engagement with residents.
• Encourages and manages engagement with team members, residents and family members.
• Works closely with the Clinical Supervisor and Team members to ensure regulatory compliance.
• Ensures focus on person-centered care.
• Responsible for the daily operations of the assigned unit.
• Manages and audits CNAs job performance and rounds every hour.
• Administers medications and treatments as ordered by a provider, while being in compliance with state and pharmacy regulations.
• Participates in wound rounds on unit's wounds with the Physician.
• Communicates all wound orders and treatments with the oncoming nurses.
• Inserts intravenous devices and assesses and maintains patency of intravenous sites.
• Identifies, reports, and documents incidents and accidents.
• Ensures problems and complaints involving nursing services are managed in a timely and appropriate manner.
