Requirements

• Active Licensed Practical Nurse (LPN) License in good standing.
• A compassionate, detail-oriented, and dedicated professional committed to excellence in resident care.

Nice To Haves

• Experience in Skilled Nursing or Assisted Living preferred.

Responsibilities

• Assist Nursingwith Physician orders and ensure they are sent out in a timely manner and signed
• Ensure new and changed orders are documented properly in PCC/EMAR
• Lab tracking
• Prepare and create new resident charts
• Maintain and update resident charts
• Keep track of ASP and DME due dates and updates with DON
• Review glucometer with PCC
• Self admin audits
• Cart audits weekly
• Assist DON with red binder audits
• Keep track and review state list and update in PCC and review with DON
• Ensure proper storage of old files and discharged residents
• Order supplies
• Weekly clinical reviews with DON
• Fall assessments in PCC and Review
• Work with transport for resident appointments appointments and ensure changes are made in PCC and families are updated.
• Assist nursing with admissions
• Community assessments as needed
• Perform additional duties assigned to support the best possible care for our residents.
