Residential Program Midland Care Connection’s residential program provides a supportive, home-like environment designed to enhance quality of life for individuals with complex health needs. Staff Nurse | LPN At Midland Care Connection’s residential program, a Licensed Practical Nurse (LPN) plays a key role in providing daily nursing care and supporting residents’ overall health and comfort. The LPN assists with medication administration, monitors vital signs and changes in condition, performs basic clinical procedures, and collaborates closely with registered nurses, physicians, and the interdisciplinary care team. They also provide hands-on support, educate residents and families, maintain accurate documentation, and help ensure care plans are followed with compassion and attention to individual needs, contributing to a safe and supportive living environment.

Requirements

  • Current Licensed Practical Nurse (LPN) license in good standing
  • Graduation from an accredited practical nursing program
  • Knowledge of basic nursing principles, medication administration, and clinical procedures
  • Ability to provide compassionate, patient-centered care to individuals with complex or chronic health needs
  • Strong communication and documentation skills
  • Ability to work collaboratively as part of an interdisciplinary care team
  • Physical ability to perform nursing duties, including standing, walking, lifting, and assisting residents

Nice To Haves

  • Previous experience in residential care, long-term care, hospice, or home health settings
  • Experience working with older adults or individuals with serious or chronic illnesses
  • Familiarity with electronic health records (EHR) systems
  • Training or experience in palliative or supportive care
  • Strong interpersonal skills with the ability to support residents and families during sensitive situations
