Provides each resident with professional nursing care, assessment and evaluations in their scope of practice. The position involves performing all licensed practical nursing functions and services as directed by the Registered Nurse to all residents. Responsibilities include completing assessments on residents, making appropriate interventions, applying knowledge, prioritizing resident care, and using the nursing process (assess, plan, implement, evaluate). The role also requires compliance with all pharmaceutical policies and procedures, ensuring that all medications/treatments are given safely and accurately in accordance with the physician’s orders. The nurse administers medications according to facility policy and is competent in the Pyxis medication administration system. Participation in the care planning process is essential, ensuring that resident care is implemented consistently and appropriately to the age of those served. The nurse initiates and updates the plan of care as appropriate, complies with standards of professional documentation, and communicates effectively to manage the delivery of resident care, ensuring quality care is provided. The role includes guiding staff in meeting the needs of the residents, participating in physician rounds, and communicating changes in resident condition to the physician utilizing the SBAR communication tool. The nurse processes and transcribes physician’s orders and implements them according to facility policy. Seeking educational opportunities, sharing knowledge with others, assisting with the orientation of new staff, investigating incidents, and facilitating conflict management while respecting diversity are also key responsibilities.
